Chronic Disease Management


Exercise Physiology is the study of the body’s acute responses and chronic adaptations to exercise, in particular for those who have chronic illness. We use this knowledge to utilise movement and exercise for the prevention and management of chronic disease or injury and assist individuals to restore their optimal physical function, health or wellness.

In essence, exercise is medicine. We use exercise to promote physical and physiological changes in the body to improve health markers such as weight, blood pressure, cholesterol and blood glucose levels, bone density and much, much more. Exercise is a powerful tool in the management of any number of chronic conditions such as:

  • Cardiovascular and Pulmonary Diseases
  • Diabetes
  • Osteoporosis
  • Arthritis (Osteo & Rheumatoid)
  • Cancer
  • Mental Health
  • Neuromuscular Conditions such as Multiple Sclerosis, Cerebral Palsy & Parkinson’s Disease


Chat with your GP, as you may be eligible for a Chronic Disease Management Plan, a government initiative which subsidises the cost of allied health services including Exercise Physiology.

What is Type 2 Diabetes Mellitus?

Type 2 Diabetes Mellitus (T2DM) is a chronic condition in which the body becomes resistant to the normal effects of insulin and over time gradually loses the ability to produce enough insulin in the pancreas. The main role of insulin is to move glucose from our bloodstream into the body's cells to make energy.

Research has indicated that there is a strong correlation with genetics and family-related risk factors (inherited genes or sharing an environment with risky health behaviours) which is often associated with modifiable lifestyle risk factors. Research has been unable to pin-point what the exact genetic cause of T2DM is. Although people may be able to significantly hinder or even stop the progression of the condition through diet and engagement in physical activity.



Causes of T2DM:

1. Family history of diabetes

2. Over the age of 55 – the risk increases as we age

3. Over the age of 45 and are overweight and/or have high blood pressure

4. Over the age of 35 and have Aboriginal or Torres Strait Islander genealogy.

5. Over the age of 35 and are from Pacific Island, Indian subcontinent or Chinese cultural background

6. If you have given birth to a child over 4.5 kg (9 lb), or had gestational diabetes when pregnant, or have a condition called Polycystic Ovarian Syndrome (PCOS)

7. Have a mother who has had gestational diabetes when you were in utero.

Managing T2DM

At this current stage there is no cure for T2DM, however, the condition can by managed through diet, exercise and/or pharmacotherapy. Effectively managing diabetes is the best way to prevent diabetes-related complications (heart disease, stroke, blindness, peripheral neuropathy, lower-extremity amputations, etc.)


360 Health Clinic’s Intervention

The team at 360 Health Clinic are offering small group classes that will be overseen and run by our Accredited Exercise Physiologists. Classes will be bulk-billed by Medicare under the item codes 81110 and 81115. We are also offering a second class that will be run by our Accredited Exercise Scientists / Personal Trainers for a cost of $12. This is not compulsory to the program, but will complement the previous session.

To find out if you're eligible for the T2DM small group session please chat to your GP and have them fill out the following Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes along with a New Client Form and send it back to healthclinic@360fitnessclub.com

INJURY REHABILITATION


Whether you’ve been injured playing sport or on the job or have recently undergone orthopaedic surgery, our AEP’s can help you get back to your best! Exercise is a crucial component in the rehabilitation journey.

Generally injury or surgery is followed by a period of ‘time off’ or a break from your usual levels of activity, resulting in physical deconditioning.

Working with an Exercise Physiologist will involve a comprehensive functional assessment and take into account any and all of your health concerns or injuries.

They will then work with you to devise a progressive treatment plan targeted towards your personal goals and keep you moving forward!



Our practitioners are registered to provide injury rehabilitation services under Workers Compensation, Comprehensive Third Party and Income Protection claims

NATIONAL DISABILITY INSURANCE SCHEME


NATIONAL DISABILITY INSURANCE SCHEME

The introduction of National Disability Insurance Scheme (NDIS) enables individuals with a disability to receive greater support and rehabilitation to manage their health, gain or maintain their independence and assist them to live fulfilling lives.

We offer services to self- and plan-managed NDIS participants with funding in the following support categories:

  • Improved Health and Wellbeing
  • Improved Daily Living

Our three clinic locations all offer wheelchair access and for those who are unable to attend our clinic we also offer home visits or Telehealth services.

Exercise Physiology is a treatment which focuses on the individual as a whole, we work closely with the participant and other involved treatment providers to help them achieve their goals through increased functional capacity and independence. Furthermore, engaging in Exercise Physiology treatment is a great way to increase their social and community participation.


Our three clinic locations offer wheelchair access and for those who are unable to attend our clinic we can arrange home visits or Telehealth services.

Aquatic Therapy


AQUATIC THERAPY

Water provides a therapeutic environment for both exercise and rehabilitation offering a wide range of benefits for people suffering from a number of health conditions including:

  • Chronic Pain & Disease
  • Orthopaedic Rehabilitation
  • Mental Health
  • Increased Body Mass

The properties of water all contribute to different benefits on the body. The buoyancy of water reduces weight bearing stress allowing free and fluid movement, often with increased range of motion. Density creates an environment that can strengthen and tone muscles while hydrostatic pressure and temperature helps to improve circulation, decreases swelling, promotes muscle relaxation and aids pain relief.

Our AEP’s can provide you with an individual hydrotherapy program for pain management, rehabilitation or general health. We also offer Gentle Aqua classes suitable for seniors, injury rehabilitation and chronic pain management. For more information see our ‘Classes’.



Aquatic therapy is available in Tamworth and Gunnedah year round, and in Quirindi during the Summer months.

STRENGTH & CONDITIONING

STRENGTH & CONDITIONING


A strength and conditioning program designed by a qualified expert provides an athlete with the capacity to to improve, excel and achieve great things both on and off the sporting arena. Through an individualised program you can expect to strengthen supporting muscles, even out muscle imbalances, correct posture, increase mobility and stabilize joints. Further to this our programs teach individuals to properly execute movement patterns, enhance coordination and peripheral skills, decrease injury risk and build strength, power and speed.


Strength and conditioning training is not limited to those pursuing athletic performance but also plays an integral role in growth and development through childhood and adolescence. Strength training promotes healthy muscle and bone development, as well as balance, coordination and body awareness.

One of the positives to come out of 2020, was the introduction to a new platform through which we could deliver our services. We indeed had to learn on the run, but quickly recognised the huge value in offering Telehealth particularly as look to increase the accessibility to health care in rural and regional New South Wales.



Telehealth is the term used to describe the delivery of health services via an online platform. We use an application called PhysiApp to conduct video consultations with you, wherever you are! As you would expect in any face-to-face consultation we will discuss your health and activity history, what you would like to achieve through treatment, perform baseline assessments and develop a comprehensive plan to keep you moving towards your goals.
